The NADAL® Calprotectin/Lactoferrin Test is a rapid chromatographic immunoassay (non-invasive assay) for the qualitative detection of human calprotectin and lactoferrin in human faecal specimens. The test is intended for use as an aid in the diagnosis of inflammatory gastrointestinal disorders and is designed for professional use only.
Calprotectin is a calcium-binding protein that makes up 5% of the total protein and 60% of the cytosolic protein of neutrophils. It has bacteriostatic and fungistatic properties and is found in faeces at levels six times higher than it is in plasma. Lactoferrin (LF) is a glycoprotein that is produced by neutrophils, mononuclear phagocytes and epithelial cells and is contained in the secretory fluids such as saliva and breastmilk. Its function consists of blocking bacterial growth by limiting the availability of iron. This effect is enhanced by the presence of specific secretory IgA antibodies directed against bacteria. LF also has a bactericidal effect by causing direct damage to cell membranes in conjunction with lysozyme. When inflammation develops in the gastrointestinal tract, neutrophils and phagocytic cells migrate to the inflammatory focus and release granules containing LF. LF is stable in faeces and is easily detected with immunochemical methods. LF and calprotectin are elevated in patients with inflammatory bowel disease (IBD). IBD, including ulcerative colitis (UC) and Crohn’s disease (CD), is associated with elevated neutrophils and represents a spectrum of diseases characterised by an idiopathic and chronic inflammation affecting the gastrointestinal (GI) tract. Pediatric and adult patients with IBD may demonstrate a variety of clinical symptoms (including abdominal pain and diarrhoea) which can be non-specific. The NADAL® Calprotectin/Lactoferrin Test is useful in differentiating organic IBD from functional gastrointestinal disease, such as irritable bowel syndrome (IBS). It is a simple, non-invasive assay that is especially useful for children, who may require general anesthesia for colonoscopy. Moreover, this faecal calprotectin detection can be used to predict a relapse.
