The AFIAS TSH test is a fluorescence immunoassay (FIA) for the quantitative determination of thyroid-stimulating hormone (TSH or thyrotropin) in human whole blood, serum or plasma. The test is used as an aid in the treatment and monitoring of thyroid disorders. For in vitro diagnostic use only.

Determination of the blood level of thyroid-stimulating hormone (TSH or thyrotropin) is recognised as an important measurement in the assessment of thyroid function. TSH is released from the anterior pituitary gland and stimulates the production and release of triiodothyronine (T3) and thyroxine (T4) from the thyroid gland, which is primarily responsible for body metabolism. TSH is a glycoprotein with a molecular weight of about 28,000 daltons and consists of two chemically distinct subunits, alpha (89 amino acids) and beta (115 amino acids). Although the concentration of TSH in the blood is extremely low, it is essential for the maintenance of normal thyroid function. The release of TSH by the anterior pituitary gland is regulated by thyrotropin-releasing hormone (TRH), which is produced in the hypothalamus. The blood levels of TRH and TSH are inversely proportional to those of thyroid hormones. When there is a high level of thyroid hormones in the blood, less TRH is released by the hypothalamus, so less TSH is released by the pituitary gland. The opposite process occurs when there is a reduced level of thyroid hormones in the blood. This process, also known as negative feedback, is responsible for maintaining the correct hormone levels in the blood.
